Case-2: A case of Dermatomyositis at Life Force

    A 2-year-old boy came to Life force on 7th April 2015 for the treatment of Dermatomyositis. His parents reported that he slowly developed red rashes and itching since August 2014. The rashes were mainly present on joints of fingers, wrist, elbow, and knees bilaterally. Gradually he started developing swelling on the face. On examination heliotrope rashes i.e. violet color rashes on upper eyelids was found to be present.

    Parents also said that he had difficulty in sitting down. Patient was unable to bend down. These symptoms indicated the involvement of different muscles. Generalized stiffness of the body experienced by the patient.

    All symptoms of dermatomyositis increased during winters, on having sweets and on stopping steroidal medications.

    Investigation reports of the patient revealed increased ESR ( 30 mm at the end of 1 hour). CPK levels were found to be normal (115 U/L). The levels of LDH was found to be very high (1072 U/L) indicating severe muscle damage.

    MRI revealed diffuse hyperintense signals at bilateral gluteal muscles and thigh muscles. Diffuse hyperintense signals were also detected at paraspinal muscles and leg muscles.

    Under allopathic treatment he was prescribed syrup omnacortil (Prednisolone) 10 ml per day along with antacids and calcium supplements.
